Adult Social Work Practitioner x2 jobs with Sheffield City Council

Adult Future Options works with adults, mainly aged 18-65, who have a physical or learning disability and are eligible for support under the Care Act.

You will work closely with these individuals, their families and support networks to promote the individual’s independence while ensuring that their health and wellbeing is maintained. The service helps people live fulfilling lives at home, where they are connected to the community and resources around them.

The purposes of the service are:

  • Enable individuals to develop their daily living skills and work towards increased independence, i.e. own tenancy in the community.
  • Work with carers, families, friends and advocacy services to ensure a person-centred approach is maintained.
  • Provide support or guidance to build and maintain healthy interpersonal and family relationships.
  • Encourage people to access the local community, supported or independent, to promote social inclusion with peers and the wider community.
  • Identify future life goals, meaningful activities, and aspirations to work toward, such as voluntary or paid employment.

Your main responsibilities will be completing annual assessments with people who normally live in residential or assisted living settings. You will consider whether the support they receive remains adequate to their level of need and order or change care packages where necessary.

Using a strengths-based approach, you will seek to empower the person to have more choice and control over their life. You will prevent and manage safeguarding issues as they arise, with the support of social workers and other professionals, as part of a multidisciplinary team. We are looking for candidates who are person-centred in their approach and who have excellent communication skills.

The role will include liaising with various teams and external suppliers, primarily via electronic communications, requiring the post holder to trust IT systems.
