He added: “We know that people with diabetes are experts on their condition and we want to hear their feedback on how we can improve our services.”

The form must be completed by May 31, NHS chiefs said.

The reference group will include online and in-person feedback as well as participating in surveys, providing feedback in a focus group discussion and engaging in future service design work with staff and clinicians.

Amanda Veall, clinical referral group leader, said: “No matter what type of diabetes you have, or whether you can only support us online or in person, we want to hear from you.

“We are also interested in people who have a family history of diabetes or who want to understand more about diabetes services.”
