Three charged with auto insurance fraud in Miami after falsifying injury claims

Authorities in Florida have charged a Miami-area nurse and two others with creating false medical reports and attempting to defraud an auto insurance carrier of more than $47,000.

Local news reports said state Department of Financial Services investigators focused on Ace Medical & Rehab Center after receiving complaints that people associated with the clinic were misleading insurance companies.

Clinic owner Yanirma Toledo, 42; medical assistant Maylett Leiva-Santana, 42 years old; and Lensky therapist Jose Hernandez-Santiago, 43, were charged this week in the alleged scheme, Miami’s Local10 TV news reported.

Investigators created their own fake accident report and had two undercover witnesses go to the clinic. The “patients” received check-ups but no actual therapy, authorities told the media. The clinic then billed insurance companies for more therapy sessions that never happened. United Auto Insurance Co. was billed $47,310 for fake injuries, authorities said.

The trio is being prosecuted for fraud, theft, insurance fraud and conspiracy charges. They were arrested Tuesday in Miami.
